Setting up forms

A common requirement for many businesses is for client's to fill in a form or signed disclaimer before their first consultation or before every consultation.  A form may be specific for a service or a common form for everyone.  It can also contain any number of your own questions.

Forms can be filled out electronically by clients on an iPad or similar device or emailed to them to fill out beforehand.

  1. Go to Settings on the sidebar.
  2. In the Products & Services section under the Forms heading click on Manage Forms.
  3. Click the +Add New button to create a new form.
  4. The Form Details screen will now appear. Enter a name for the form and add a category if necessary.Choose when you want this form to be used. If you want to use the form for a specific service, please see our guide on setting up Service Forms.
  5. Select which fields you want to display on the form. If you have ticked the Signature box the client will be required to sign on screen when submitting the form.
  6. Select which communication options you want displayed on the form.
  7. If you like, you can change the labels on the fields displayed on the form. In this example we've changed the Client First Name field to display on the form as "Given Name(s)", and Client Last Name to display as "Surname".
  8. You can optionally write some text that will be displayed at the top of the form, this might be something like a disclaimer.
  9. Click the Save button at the bottom of the screen.
  10. The Questions tab will now be visible. Here you can add any other questions you want to be filled out on the form, for example, allergies or preferences. 
  11. Click the Add a new Question button to add a question. There are various question types:

    Plain Text. The client will be able to write a free text answer. 

    Plain Text Multi- Line. The client will be able to write a free text answer over multiple lines.

    Date. The client will be able to select a date. 

    Number. The client will be able to enter a whole number such as 1, 3 or 50. 

    Decimal Number. The client will be able to enter a decimal number such as 2.6 or 11.87. 

    Drop Down List. The client will be able to select one answer from a list of answers you enter in the box.

    Yes or No. The client will be able to choose yes or no. 

    Multiple Select. The client will be able to tick any number of the answers you enter in the box.

    Text Block. You can use this to add a block of text (such as a disclaimer) that does not require any client input.

    File Upload. The client will be able to upload any required files such as images or scanned documents
  12. Once you have entered all your questions you can move them up or down to change the order they are displayed to the client.
  13. You can preview your form by clicking the Preview Form button.
  14. You can click on the Back To Forms button to add another form if required.
